Mako 10 Middleware exception '$body must not be accessed before initialization'

I have upgraded all my apps to Mako 10. I am getting this exception now with Middleware. I can't see what it wrong with my code, any suggestion?

I'll release a fix for this. In some cases the response body doesn't get a value before being read and it causes this issue.
