Add scoped assignment, severity, comments and dispositions with revision conflicts. Keep the original decision immutable and persist opt-in signed callback intent for durable handoff. Add dashboard and CLI management.
Validation: Review attribution, duplicate-write conflicts and viewer restrictions passed; later integration PR extends signed callback coverage.
