feat(claude): Plan Creator Skill

Closes #1209

Summary

• Before: No skill for turning an agreed SRD/spec into an implementation plan.
• Now: Added a plan-creator skill (.claude/skills/plan-creator/SKILL.md) that generates implementation plans grounded in docs/wiki and the domain-map blast-radius procedure — the step between "spec agreed" and "start coding".
