docs: replace internal Phase A/B naming with plain language - #52

"Phase A/B" came from the internal agent-integration design plan and means nothing to external readers. Example README now says what each thing is (Base MCP plugin = user approves each tx in Base Account; agentkit = autonomous agents with their own wallet) and drops a duplicated See-also bullet. Base MCP plugin README inlines the actual fill-order rationale instead of citing "design plan Phase A.6".

Companion to thetanuts-agentkit#3. "Phase A/B" is internal design-plan jargon — external readers can't know what it means. Replaced with plain descriptions (user-in-the-loop Base MCP plugin vs autonomous agentkit), removed a duplicated See-also bullet in the example README, and inlined the fill-order exclusion rationale in the Base MCP plugin README instead of citing "design plan Phase A.6".
