This merges Sams changes to the github actions extension https://github.com/aha-develop/trigger-github-actions/pull/2 into this Claude focused extension.
This gives us the ability to target a workflow - i.e. claude.yml or mention @claude ( existing behaviour )
It also gives a much nicer flow where a PR is created for the work directly, instead of the old issue creation behaivour
