The README described a harness nobody could see. Two clips from one task — run end to end against a repository the harness had never seen, a small note store asked to reject empty titles.

First screen: stage 6. It carries two claims at once, which is why it goes at the top:

✗ an empty title is refused
  AssertionError [ERR_ASSERTION]: Missing expected exception.

MCP server "plugin:harness:stages" requests your input
Stage 6 — approve the failing tests for reject-empty-note-titles?
They ran and failed as intended. Accepting moves on to writing
the code that makes them pass. Decline to keep working on the tests.
  ❯ Accept Decline

The tests exist before the feature and fail for the reason they were meant to, and the question is the plugin's wording — Claude neither phrases it nor sees the answer. The clip runs on through the implementation to All 10 pass, including the test that existed before. No test was edited or weakened.

Second: stage 5, beside the paragraph that describes it.

Status note now says two tasks rather than one. The second was on another project, and no source file existed before the design was approved — visible in that run's branch, where only docs/ appeared until stage 5.

Case 54 holds every local path the README points at. A broken image in the first screen is the most visible way for this project to say something untrue about itself, and it was the one kind of rot nothing here would have caught. Verified it fails before trusting it.

61/61 tests pass. Assets are 1.18 MB and 1.06 MB, within GitHub's 10 MB image limit.

Two clips from one task, run end to end against a repository the harness had
never seen — a note store, asked to reject empty titles.

The first is stage 6, and it earns the first screen because it carries two
claims at once: the tests are written before the feature and fail for the
reason they were meant to, and the question you are asked is the plugin's
wording, not Claude's. The second is stage 5, next to the paragraph that
describes it.

The status note now says two tasks rather than one, since the second was on
another project and no source file existed before the design was approved.
