Provide support for okta auth method

For the most part, this is identical to the ldap auth method, but the mount point defaults to okta.

Note: This probably requires #197 to be addressed first.

Blocked by #198. Once custom mount points are supported, adding Okta support would be trivial (either a new method or just documentation showing LDAP with custom mount).
