fix: remove redundant automation confirmation

Summary

• Remove the plugin-specific confirmation guard for Chrome and Computer Use in TUI, RPC, and print modes.
• Keep authorization with OMO permission presets and rules; preserve native and Chrome elicitation behavior.
• Update regression coverage, host QA, and permission documentation.

Verification

• Regression failed before the fix: the TUI confirmation was invoked 11 times.
• bun run check: TypeScript and all 240 tests passed.
• bun run qa:host: OMO_HOST_QA_PASS.
• npm pack --dry-run passed.
• Independent source review approved commit 338ca79.
• No live browser or TUI smoke test was performed for this change.
