- Bump version badge and footer 2.17.0 → 2.18.3 - Add v2.18.3 entry to Recent Releases section - Add full CHANGELOG entry covering all three commits in the release: duration suffix, /explain import inlining + grounding rules, and the perf tuning (import ranking, tighter prefill caps, lower maxTokens default).

Summary
Docs-only follow-up to PR #84. Brings README + CHANGELOG up to v2.18.3 so the GitHub repo page accurately reflects the released version. Marketplace listing stays on the 2.17.0 README baked into the v2.18.3 .vsix until the next feature release.
