feat: update memory backend, sync UI state, and standardize documentation

Overview

This PR synchronizes the latest local improvements with the upstream repository. It includes essential updates to the memory storage logic, UI state management, and a comprehensive overhaul of the project documentation.

Key Changes

🛠️ Backend & Logic

• Memory Store Optimization: Refined the SQLite FTS5 search logic and memory CRUD operations.
• REST API Enhancements: Updated the Express router to handle partial updates and error states more robustly.
• MCP Integration: Standardized tool registration for memory_list, memory_read, memory_create, and memory_search.

💻 UI & Frontend

• State Synchronization: Improved the useMemories hook to better handle debounced searches and tag filtering.
• Component Refinement: Finalized the React 19 visual explorer components.
